CAR WRECK?  NEED A "BLUE" FORM FOR A MINOR ACCIDENT? 

The Police Department gets requests every day for a "Blue" form, or state accident form. Here is a .PDF version of that form that you can fill out yourself. This form is for minor accidents where in most cases the vehicles have been removed from the accident site, and your insurance company requires that you file a report with the state.

 If there is injury or death in any accident, you must report it to your local police or sheriff by the quickest means of communication. If you are in doubt of whether you need to report the accident, call your local police or sheriff immediately.
